Referrals Co-Ordinator job vacancy in Waterlooville, near Portsmouth

We’re recruiting a Referrals Co-ordinator to provide high quality comprehensive and confidential administrative support to the referrals officer in the form of processing, and tracking referral information and to provide data and reports.

Cornerstone Healthcare is a team of healthcare professionals that operate currently across Curdridge and Waterlooville in Hampshire, specialising in neurological degenerative illness with complex mental and physical health issues and challenging behaviours.

Duties include…

  • To provide effective, timely and confidential administrative support
  • To ensure that relevant records and documents are kept up to date and stored securely and confidentially at all times
  • To respond timely and effectively to all referral related enquiries
  • Administer the CRM database of commissioners, funders, referrers and their enquiries, paying particular attention to updating prospective client records
  • Administer the referral pipeline, ensuring it is accurate, up to date and distributed to the appropriate personnel at the designated time
  • To maintain the daily and weekly referral reports ensuring accurate data is being recorded and presented in a timely manner
  • To inform GMs when the funding has been approved so arrangements can be made
  • Keep in regular contact with all internal stakeholders (both geographically and office based) to ensure an effective flow of communication
  • Support GMBD with diary management and meeting planning
  • Participate in training and take personal responsibility for personal development
  • To work to and exhibit the values of the company and maintain standards of behaviour in accordance with agreed policies and codes of conduct
  • To operate as a call handler for all referral activity ensuring detailed and accurate messages are received and appropriate information relayed
  • To stay abreast of service placement activity, and regularly liaise with managers with regards to placement activity
  • To provide relevant information to placing authorities in the interest of securing a referral ensuring no delays in the process.
  • To provide absence cover for the Referrals Officer
  • To provide detailed handover information regarding purchasing authority/referral activity to the Referrals Officer
